From the systems implemented in 2014 results in stakeholder satisfaction surveys, showed that service delivery improved by 10%. Risks are not always obvious and it is essential to be prepared to mitigate the risks in all service provision.  Healthcare Dynamics have designed a risk matrix that is easy to populate and it allows the awareness and there for the mitigation of adverse events. Being prepared for all hazards natural or other is the proactive way to stay afloat.

DoH Core Standards

The importance of providing quality health services is non-negotiable. Better quality of care is fundamental in improving South Africa’s current poor health outcomes and in restoring Patient and staff confidence in the public and private health care system. If quality is defined as “getting the best possible results within available resources”, then these National Core Standards set out how best to achieve this. Dr Aaron Motsoaledi (MP)


Legislation and compliance to the basic standards as laid out in the Department of Health Core Standards is essential. All Healthcare service institutes have to be compliant in most aspects and given the complexity of the audit experience is adventitious when planning and measuring to be compliant.

Root cause analysis

Time line analysis and the question “why” is the only process that seeks the truth of the cause of any adverse event. If we do not know form where we are coming we will have no idea to where we are going. Based on this it is good governance and best practice to analyse adverse events to prevent future events that negatively affect outcomes.

By three methods we may learn wisdom: First, by reflection, which is noblest; Second, by imitation, which is easiest; and third by experience, which is the bitterest.

Medico/legal Forensic file auditing

The complete picture and events pertaining to the client are woven in the charts and reports.  Insight is required to draw all the information from the documentation summarise and ensure it is clear and valid for the purpose that the file is being audited.  Evidence based fact and research may be required to validate the conclusions required to proceed with an enquiry, expert witness to attest to the facts sourced and or insight shed on the actual events.  In all instanced Healthcare dynamics consultants will provide clear concise information with evidence as required by the client.
